Please consult with the Registrar’s Office or the Shared Services Center to determine which documents you will need to make the update. Primary Name (Legal Name) Date of Birth (mm/dd/yyyy) Birth Location, Birth Country, Birth State Gender National ID: U.S. Social Security Number Country of Citizenship Citizenship Status: Citizenship Status in the United States Visa/Permit Data Example Scenario A former undergraduate of the University of Michigan Ann Arbor campus who has legally changed their name after graduation and plans to apply to a master’s program within Rackham should first update their name in the university’s records before submitting their application for admission. Supporting Gender Inclusivity Back to Top The Rackham admissions application now includes questions allowing prospective graduate students to provide their pronouns, select from an expanded list of gender identities, and submit the preferred name that they would like to be used when being addressed. Responses to the sex question will be removed from application materials generated by Rackham for admissions committees; if an applicant enters a preferred name, the preferred name will be used in lieu of the response to the first or given name question. These changes were made to support the gender inclusivity of the Rackham application and the efforts of programs to matriculate the students whom they admit.
