Rackham – Ann Arbor Program Application Forms
Apply to a Rackham Program Purpose of the Application Application Fee
Rackham Graduate Program Application

To apply for a degree or certificate program. The use of this form is required of prospective students who seek to become a Rackham degree student for the first time.

The form is also required of current or former Rackham students who seek to change from one graduate program to another within the graduate school.

Current PIBS, SIDP, or Ph.D. students changing to a master’s in the same program see the Change of Status Application in the chart below. The Change of Status Application does not require a fee.

  • $75 – The application fee for United States citizens and those with Permanent Resident status.
  • $90 – The application fee for non-United States citizens.
  • $10 – The application fee for current Rackham students, regardless of citizenship.
  • The application fee is paid by credit card and is non-refundable.
Readmission Application To apply to re-enroll in the same program as a master’s, certificate, or Doctor of Musical Arts (D.M.A.) pre-candidacy student. No application fee
Reinstatement Application To apply to be reinstated in the same Ph.D. program for students who withdrew or were discontinued from a Ph.D. program. No application fee
PIBS Declaration of Ph.D. Program Application

Current first-year Rackham PIBS program student seeking enrollment in one of Rackham’s 14 doctoral programs of specialization (e.g., Admitted to the PIBS program this past fall and seeking enrollment in the Rackham doctoral program in biological chemistry).

  • Must be a current Rackham PIBS student.
  • Must complete one academic year (fall and winter) in the PIBS program prior to the term of application.
  • Must meet with the program advisor of the Rackham Ph.D. program of application prior to submitting the application.
  • Must submit to Rackham Admissions an official and final U.S. or equivalent bachelor’s degree academic record prior to submitting the application.
  • If you received your undergraduate degree from U-M Ann Arbor, you do not need to request the Registrar to send an official transcript to Rackham.
  • You are not required to submit an application fee.

Dual Admission Application To apply to add a degree or certificate program.
  • $10 – The application fee
  • The application fee is paid by credit card and is non-refundable.
Dual Undergraduate/Graduate Study Supplemental Forms

These forms must be completed in addition to the Rackham Graduate Program Application.

Required when applying for sequential or concurrent dual status in an undergraduate and graduate master’s program.

Non-Degree Graduate Study
Apply for Non-Degree Graduate Study Purpose of the Application Application Fee
Rackham Departmental Non-Candidate for Degree (NCFD)
  • Rackham Unclassified Admission: To apply for admission to enroll for one term only. Enrollment is limited to two courses total, only one course per department or program. On the application form, applicants must choose the option Rackham Unclassified.
  • Departmental Non-Candidate for Degree (NCFD) Admission: To apply for admission to enroll for two terms only in two or more courses in a single program. On the application form, applicants must choose the option Department NCFD (Non-Candidate for Degree).
  • Graduate department or program approval is required before applying.
  • $75 – The application fee for United States citizens and those with Permanent Resident status
  • $90 – The application fee for non-United States citizens
  • The application fee is paid by credit card and is non-refundable.
Spring or Summer Guest Application To apply for admission as a spring or summer semester guest student for institute, workshop, and short course participants. No application fee
Ann Arbor Guest Application To apply to enroll in a graduate course on the Ann Arbor campus if you are a Rackham graduate student currently enrolled on the Dearborn or Flint campuses. No application fee
Big Ten Academic Alliance Traveling Scholar Application To apply for permission to take courses at other Big 10 Universities or the University of Chicago. No application fee
Michigan Intercollegiate Graduate Studies (MIGS) Program Application To apply for permission to take graduate courses at another institution, in the State of Michigan, which participates in the MIGS program. No application fee
Rackham – Dearborn and Flint Applications
Apply for Non-Degree Graduate Study Purpose of the Application Application Fee
Dearborn Guest Application To apply to enroll in a graduate course on the Dearborn campus if you are a student currently enrolled on the Ann Arbor or Flint campuses. No application fee
Flint Guest Application To apply to enroll in a graduate course on the Flint campus if you are a student currently enrolled on the Ann Arbor or Dearborn campuses. No application fee

Hours: Admissions is open Monday through Friday, 9:00 a.m. to 5:00 p.m. The office is closed Saturdays and Sundays and on university holidays.
